Rhinoplasty is the most requested facial aesthetic procedure worldwide, and Turkey performs more rhinoplasty than any country in the world. That is both an opportunity and a responsibility. The opportunity: extensive surgeon experience and access to high-quality care at significantly lower cost than the United Kingdom, North America, or much of Europe. The responsibility: choosing a surgeon whose volume on this specific procedure is matched by careful, individualised planning for every patient.

I am Op. Dr. Aytaç Kılıç, an ENT and facial aesthetic surgeon based in Antalya, with international surgical experience gained at Harvard Medical School’s Department of Otolaryngology. Rhinoplasty — both primary and revision — is the core of my practice, performed week after week on patients from the UK, Europe, and beyond. The goal is a result designed for your face, not pulled from a template.

What Rhinoplasty Is

Rhinoplasty — surgical reshaping of the nose — addresses both aesthetic and functional concerns. The procedure can refine the bridge, reshape the tip, narrow the nostrils, correct asymmetry, and improve breathing through the same operation. As an ENT surgeon, I evaluate the breathing aspect with the same seriousness as the aesthetic one. A nose that looks beautiful but breathes poorly is not a successful surgery.

Modern rhinoplasty has moved away from aggressive reduction toward structural preservation. Twenty years ago, surgeons removed cartilage and bone freely. Today we know that long-term results — both aesthetic and functional — are best when natural structure is preserved and refined rather than removed. This shift in philosophy is the most important development in the field.

Bespoke Design for Each Patient

There is no single “house style” applied to every patient in this practice. Some patients want a refined, undetectable result. Others — particularly those combining rhinoplasty with fox eye surgery or upper eyelid procedures — want a more defined, intentional look that reads as deliberate and elegant. Both are valid aesthetic choices.

What we believe in is designing each nose for the specific face it belongs to. Before any surgical plan is finalised, four factors are weighed together: your facial anatomy and proportions; your skin type and thickness, which dictate what definition is realistically achievable; your stated goals and reference images; and the surgical techniques that can be delivered predictably on your structure. We tell you honestly when your request is not realistic — and we adjust the plan together.

Primary vs Revision Rhinoplasty

If this is your first rhinoplasty, you are seeking what we call primary rhinoplasty — surgery on a previously unoperated nose. Primary cases work in natural tissue planes with the full anatomy intact. They are technically demanding but predictable.

If you have had previous rhinoplasty and are unhappy with the result, you may be seeking revision rhinoplasty — a separate, more complex topic that we address on its own page. Revision is technically harder, takes longer to heal, and requires specific revision experience.

Surgical Approaches

Open vs Closed

Open rhinoplasty involves a small incision across the columella (the strip of skin between the nostrils), allowing the surgeon to lift the skin and visualise the underlying structures directly. Closed rhinoplasty places all incisions inside the nostrils, with no external scar, but provides less direct visualisation.

There is no universally “better” approach — there are appropriate approaches for specific cases. Open is generally preferred for cases requiring significant tip work, asymmetry correction, or revision. Closed can be ideal for straightforward bridge work in patients with predictable anatomy. The columella incision in open rhinoplasty heals to be essentially invisible within months.

My approach: I use both techniques and choose based on the specific case, not on personal habit or marketing positioning.

Ultrasonic Rhinoplasty

When bone reshaping is required, I use ultrasonic instruments rather than traditional rasps and chisels. Ultrasonic technology selectively cuts bone without damaging surrounding tissue, leading to less bruising, less swelling, and more precise reshaping — particularly important for refinement work where millimetres matter.

Structural Preservation Rhinoplasty

When the patient’s anatomy allows, I prefer structural preservation techniques — reshaping the nose by repositioning structures rather than removing them. This approach produces durable results and significantly reduces the risk of long-term issues like over-reduction or breathing problems.

Are You a Candidate

Rhinoplasty is appropriate for patients who are 18 or older with stable facial growth; have realistic expectations; are in good general health, with no untreated medical conditions affecting healing; are non-smokers, or are willing to stop smoking around the surgery; and understand that final results take 12–18 months to fully settle.

We deliberately turn patients away when their goal is a specific celebrity nose that does not suit their face; when they show signs of body dysmorphic disorder (BDD) that require psychological support, not surgery; when their anatomy does not allow for safe achievement of their stated goals; or when they are seeking surgery to please someone else rather than themselves.

The first consultation is partly about whether you are a candidate. It is not a sales meeting. If we do not believe surgery is the right choice for you, we will tell you, even if it costs us the case.

The Procedure

Rhinoplasty is performed under general anaesthesia. The operation typically takes 2 to 4 hours, depending on complexity.

The general sequence: pre-operative review and final marking; anaesthesia administration; incisions placed (open or closed depending on approach); reshaping of bone, cartilage, and soft tissues; functional refinement (septum, valves) if needed; closure with absorbable sutures; external splint and internal supports placed.

Patients spend one night in the hospital under monitoring before discharge. The internal silicone splints are removed at day 2 or 3. The external splint is removed at day 6 or 7.

Recovery — Honest Timeline

Days 1–3: peak swelling. Discomfort more from nasal congestion than pain — relieved when the internal silicone splints are removed at day 2 or 3. Cold compresses help. Sleep elevated.

Days 4–7: bruising fades. The external splint is removed at day 6 or 7. The nose still looks dramatically swollen — this is normal.

Weeks 2–4: most patients return to office work and social activities within 10–14 days. Visible swelling continues to subside. Strenuous exercise should wait.

Months 1–3: approximately 60–70% of swelling resolved. Initial shape becomes apparent.

Months 6–12: refinement continues. Tip swelling — particularly in patients with thicker skin — resolves last.

Final result: 12 months for most patients, 18 months for thicker-skinned patients. Our follow-up runs in parallel for approximately 18 months — weekly photos and videos in the first month, transitioning to a structured longer-term schedule. Based on what we see, we recommend tapings, massage techniques, medications, vitamins, or creams as your healing requires. This is not optional aftercare; it is the second half of the surgery.

For International Patients

Stay Duration: We recommend 7–10 days minimum. Surgery, recovery, splint removal, and final examination must all occur before flight clearance.

How long does rhinoplasty take?

The surgery itself: 2–4 hours. The full recovery to final result: 12–18 months.

Is rhinoplasty painful?

Most patients describe the post-operative experience as uncomfortable rather than painful. Nasal congestion is more bothersome than actual pain.

Will I have visible scars?

Closed rhinoplasty: no external scars at all. Open rhinoplasty: a small columella scar that fades to nearly invisible within 6–12 months.

Will I need to have stitches removed?

No. Closure is performed with absorbable sutures, so there are no stitches to remove afterwards. The internal silicone splints are removed at day 2 or 3, and the external splint at day 6 or 7; the sutures themselves dissolve on their own.

How long until I can return to work?

Office work: 10–14 days. Public-facing roles: closer to 14–21 days. Strenuous physical work: 4–6 weeks.

Is the result permanent?

Yes. Rhinoplasty results are essentially permanent. Aging continues normally on top of the surgical change.

Can rhinoplasty improve my breathing?

Yes, when functional issues are part of the operation. As an ENT surgeon, I address breathing problems with the same attention as aesthetic ones.

Will I look like myself?

That depends on what you are asking for. If you want a refined result no one can identify as surgical, we can plan for that. If you want a more defined, intentional change, we plan for that instead. Both are valid goals — provided your anatomy supports the result you are asking for.
